Associate Membership Application Form

The annual £5 membership fee covers one household of up to two adults and their children under 16 years. Annual membership runs from 1st January each year.
The information you provide will be used in accordance with the Data Protection Act (1998) and for FoMP to administer memberships, contact you and invite you to activities/events. We may share this information with Derby City Council.
